La croissance périmétrique du maxillaire et sa sollicitation fonctionnelle ou : du bon usage du Bioactivateur

Résumé

Le maxillaire est le résultat du remaniement et de l'ajustement d'une mosaïque de structures osseuses sollicitée tout au long de la croissance.

La sollicitation s'exprime au niveau des sutures bordant le prémaxillaire, l'hémimaxillaire et l'hémipalatin.

La croissance périmétrique est liée au développement du prémaxillaire. Elle peut être marginale ou apicale, influençant ainsi la position incisive.

L'action orthopédique sur le prémaxillaire, par sollicitation fonctionnelle ou activation mécanique, est possible même après l'apparition des canines permanentes.

Les appareils utilisés sont soit le Bioactivateur, soit le Biopart combiné à un arc transpalatin modifié.

L'action fonctionnelle doit être précoce. Elle n'est possible qu'en présence des canines temporaires. Il est donc indispensable de les conserver et de débuter le traitement dès la définition des troubles fonctionnels.

L'indice Idyll permet d'évaluer l'importance de la dysfonction et le moment de notre intervention.

Les résultats de travaux antérieurs et la présentation d'un cas illustrent la démarche thérapeutique.

Abstract

The maxilla is composed of a mosaic of structures which are submitted to readjustment during the different periods of growth. The functional stimulation acts particularly along the sutures boarding the premaxilla as well as the maxillary and palatal bones. The peripheral growth necessary for the correct alignment of the premaxilla. It expresses itself on the upper (apical) or lower (marginal) parts of this bone thus influencing the position of the incisors.

The orthopedic (non extraction) therapy in the upper jaw by means of functional or biomechanical procedures depends on the erupting path and situation of the permanent cuspids.

The appliances which could be used for reaching this aim in the Bioactivator or the Biopart combined to a modified transpalatal bar.

The stimulating action of the tongue on the premaxilla should take place early when the deciduous canines are still present. It is therefore compulsory to keep them and to start the treatment as soon as a functional trouble revealed.

The Idyll index allows to evaluate the importance of the dysfunction and the starting time of the therapy.

Results from previous research works and a case presentation will illustrate this therapeutical approach.
