Premier maillon de la chaîne ventilatoire, la pyramide nasale ne s'individualise que chez l'homme.
Son aspect est lié aux facteurs climatiques, et en particulier à la température et à l'humidité. Il existe une variabilité inter et intra-raciale, ainsi qu'une typologie familiale dont les thérapeutes devraient tenir compte.
