Résumé

L’épaississement gingival et les tentatives de recouvrement radiculaires sont des techniques chirurgicales à mettre en oeuvre avant le traitement orthodontique et après évaluation du diagnostic parodontal selon des indices cliniques précis et reproductibles. Le cas clinique proposé dans cet article est une patiente adulte de 53 ans nécessitant un traitement orthodontique et présentant des récessions gingivales de classe III de Miller de 13 à 23 et sur 32 et 42. Le biotype parodontal de la patiente est fin.

Le but de l’intervention développée dans ce travail est d’épaissir le parodonte de la patiente et de tenter de recouvrir ses récessions. La technique chirurgicale choisie est celle de l’enveloppe modifiée avec apport de tissu conjonctif.

Les résultats à 18 mois post-opératoire montrent un taux de recouvrement satisfaisant et une amélioration esthétique significative.

Abstract

The soft tissue thickening and root coverage are necessary surgical procedures to be realized before starting orthodontic treatment. The appreciation of soft tissue has to be evaluated during the periodontal diagnosis and follows accurate and reliable clinical assessments. The case report proposed in the article is the one of a patient of 53 years old of age presenting gingival recessions of Miller class III from teeth 6 to 11 and from teeth 22 to 27. The patient has committed to an orthodontic treatment. The periodontal biotype is thin.

The goal of the treatment exposed in the article is to thicken the soft tissue and to try to cover the roots of the teeth involved in the gingival recessions. The technique of choice is the modified envelop with insertion of connective tissue.

The 18 month post-operative results show an excellent rate of root coverage and a significant improvement of aesthetics.
