Un rapport intercanin potentiellement destructeur : la fonction retardée

Résumé

La fonction retardée est un rapport intercanin souvent négligé lors de l'examen occlusal mais dont les conséquences physiopathologiques peuvent être considérables.

Après des rappels succincts concernant le guidage mandibulaire en latéralité, le tableau clinique de cette pathologie occlusale est décrit tant au niveau symptomatologique qu'au niveau des conséquences pathologiques.

Les voies thérapeutiques sont différentes selon l'âge du patient :

- chez l'enfant et le jeune adolescent, l'action sera essentiellement préventive du fait du potentiel adaptatif des structures articulaires ;

- chez l'adulte, le collage d'un élément artificiel le long de la face palatine de la canine supérieure permet de rétablir un guidage latéral harmonieux et de soulager rapidement et efficacement le patient.

Abstract

Progressive disclusion is a canine relationship which is too often neglected during occlusal examination, but its physiopathological consequences can be desastrous.

After short review of lateral guidance mechanisms, clinical features of the occlusal pathology are described (symptoms and pathological onsets).

Therapies have to be different regarding the age of the patient :

- for the child or the teenager, prevention is the main effort, helped by a strong T.M.J. potential of adaptation ;

- for the adult, an artificial acid-etch riser on the upper canine slope contributes to the establishment of a better lateral guidance and a quicker and more efficient relief of the patient.
