Correction of an adult anterior open-bite case through a multidisciplinary approach

Résumé

Il s'agit, chez une patiente adulte, du traitement multidisciplinaire d'une béance incisivocanine associée à une biproalvéolie et une macroglossie sur un terrain ayant présenté une parodontite juvénile localisée.

Le traitement s'est déroulé en trois étapes :

- étape parodontale : phase initiale de détartrage-surfaçage associée à une antibiothérapie ayant permis de stabiliser la maladie parodontale ;

- étape chirurgicale : glossectomie losangique médiane ainsi qu'une génioplastie d'avancée qui ont amélioré l'équilibre fonctionnel labio-mentonnier et lingual ;

- étape orthodontique : après extraction des premières prémolaires maxillaires et des incisives centrales mandibulaires, la correction de la malocclusion (verticale et antéro-postérieure) a pu être réalisée.

Abstract

A multidisciplinary approach was implemented in an adult case of anterior open-bite combined with a biprotrusion, a macroglossia and a passed dental history of a localized juvenile periodontitis.

Treatment included three phases :

- periodontal phase: the initial scaling and root planing were associated with an antibiotherapy and led to the control of the periodontal disease;

- surgical phase: a glossectomy combined with an advancing genioplasty improved the labiomental and lingual functions;

- orthodontic phase: the correction of the malocclusion (with its horizontal and vertical components) required the extraction of the maxillary first bicuspids and the mandibular central incisors.
