Do Class II and Class III elastics have an effect on the temporo-mandibular joint? A study carried out using the finite element analysis

Résumé

L'implication possible du traitement orthodontique, et tout particulièrement de l'utilisation des élastiques intermaxillaires, dans la genèse des dysfonctions des articulations temporo-mandibulaires, est un des thèmes de débat classiques en orthodontie contemporaine. Dans cette étude, nous voulons donner une vue d'ensemble de la distribution des tensions exercées sur les articulations temporo-mandibulaires, lors de l'application d'élastiques intermaxillaires. Pour cela, nous avons réalisé un modèle bidimensionnel de simulation par la méthode des éléments finis. D'après nos résultats, les élastiques intermaxillaires de classe III provoquent peu de tensions sur les articulations temporo-mandibulaires. Au contraire, les élastiques de classe II entraînent des tensions plus importantes sur la zone articulaire et pourraient ainsi avoir un rôle dans la genèse de certains désordres articulaires.

Abstract

The possibility that orthodontic treatment, and especially the use of intermaxillary elastics, can play a role in the development of temporo-mandibular disturbances continues to be a major subject of debate in contemporary orthodontics. In this study, we wanted to take an overall look at the forces applied to the temporo-mandibular joint when patients wore intermaxillary elastics. To do this we created a two-dimensional simulated model using the finite element analysis. Our findings showed that Class III elastics exerted only slight force on the temporo-mandibular joint. But Class II elastics, on the other hand, led to relatively serious tension in the articulating zone and could, therefore, play a part in the development of certain articular disorders.
