Abstract
Artificial intelligence (AI) is reshaping clinical practice beyond technical performance, by transforming the cognitive architecture of medical decision-making.
This article explores the challenge of preserving clinicians’ cognitive sovereignty in increasingly data-driven environments. It highlights the tension between the implicit, intuitive nature of clinical expertise and the need for explicit verbalization imposed by current AI systems.
Three adaptive trajectories are identified, among which only an integrative approach allows effective human–AI collaboration.
The “butterfly allegory” is introduced as a cognitive framework designed to extend expert intuition without disrupting its natural dynamics.
AI is thus conceptualized as an amplifier of clinical judgment, provided it respects the functional organization of expert cognition.
These insights open new perspectives for the development of augmented medicine, combining technological efficiency with cognitive integrity.
