Résumé
La vie civilisée aggrave les dysharmonies dento-maxillaires. Les faces proximales et occlusales des dents ne s'abrasent plus comme le faisaient celles de nos ancêtres, le développement des arcades est handicapé par la baisse de l'activité masticatrice et la fréquence de la ventilation orale, pendant que la supraclusion réduit la taille de l'arcade mandibulaire.
Tous les procédés qui peuvent contribuer au développement des arcades à l'âge favorable doivent être appliqués. Si ce développement s'avère insuffisant ou instable, il faut bien se retourner vers les dents. Le traitement le mieux adapté au diagnostic de «macrodontie» est la réduction des faces proximales. Les malpositions seront ensuite corrigées et l'occlusion équilibrée.
Abstract
Civilized life aggravates arch length discrepancies. Unlike the teeth of our stone age ancestors, our teeth are only slightly affected by occlusal and inter-proximal attritional wear. In addition a lessening of masticatory activity and the frequency of nasal obstruction impede the development of our dental arches while a widespread tendency to overbite has reduced the size of the mandibular arch.
To counteract this deleterious trend, orthodontists should employ every method that can contribute to healthy, age-appropriate development of their patients' dental arches. If this approach does not fully succeed, or appears unstable, orthodontists should next direct their attention to the teeth, using as their first therapeutic tool the technique best suited for the diagnostic category, macrodontia: interproximal stripping. After this is completed, they should correct any remaining tooth irregularities and equilibrate occlusion.
