Un retard important d'éruption d'une première molaire (M1) ou d'une deuxième molaire (M2) est un signe inquiétant, préfigurant un traitement difficile. Faut-il s'alarmer face à des rétentions, voire des inclusions de secondes molaires assez fréquentes en fin de traitement ?

Une recherche des causes de ce retard fait partie du diagnostic initial ou d'une réévaluation en cours de traitement. Doit-on faire face à une rétention ou à une inclusion ?

Après avoir rappelé l'étiologie de ces problèmes, l'article relate plusieurs attitudes thérapeutiques face à des rétentions ou face à des inclusions.

Une revue de la littérature décrit les principales techniques employées pour les redressements de molaires. L'auteur s 'attache plus particulièrement à décrire sa technique face à l'inclusion d'une molaire, M1 ou M2, mais dotée d'un potentiel d'égression guidée.

Son expérience clinique est transmise à partir de plusieurs cas, ce qui permet de mieux cerner les possibilités de la technique pour la mise en place sur l'arcade de ces molaires incluses ou mésioversées.

Une approche plus précise de la mesure de l'encombrement postérieur semblerait limiter ces problèmes qui engendrent un allongement notable de la durée du traitement, mal vécu par le patient.

Abstract

An important delay of eruption of a first molar (M1) or of a second molar (M2) is a worrying sign, preceding a difficult treatment. Is it necessary to be alarmed in front of retentions, even rather frequent later erupted second molars at the end of the treatment?

An investigation into the reasons of this delay forms part of the initial diagnosis or a revaluation in the course of treatment. Do we have to face with a retention or with an inclusion?

After having to point out the etiology of these problems, this article describes several therapeutic attitudes in front of with retentions or in front of inclusions.

A review of the literature describes the main techniques employed for the uprighting of molars. The author is more particularly tied to describe her technique in front of with the included molars, M1 or M2, with a potential of guided eruption.

Her clinical experiment is transmitted starting from several cases, which permit to better determine the possibilities of this technique for the installation on the arcade of these molars included or mesialy impacted.

A more precise approach of the measurement of the posterior crowding would seem to limit these problems which generate a notable lengthening of the duration of the treatment, badly lived by the patient.
